Bilingual Clinical Medication Specialist

Job Description:

A company is looking for a Bilingual Clinical Medication Reconciliation Specialist.

Key Responsibilities

Conduct patient interviews and create comprehensive medication lists
Coordinate clinical service visits between pharmacists and members/caregivers
Manage outreach and care coordination for members enrolled in chronic care management

Required Qualifications

Current Medical Assistant (CMA) certification
Fluency in English and Spanish, Russian, or Cantonese/Mandarin
At least two years of experience as a medical assistant, with one year in medication reconciliation
Knowledge of chronic care management processes
Ability to work Monday - Friday, 9:00 am - 6:00 pm EST
