  Responsibilities
  
  The Intensive Community Wrap Around Supports program is designed in partnership with the Department of Developmental Services and the Department of Mental Health to create intensive, in-home and community wrap-around services to support adults between ages 18 and 30 with Autism Spectrum Disorder and Serious Mental Illness, or other significant mental health challenges. The goal of this program is to maintain individual stability and effective functionality for people served in their homes and communities by providing clinical services and coaching supports to identify and respond to barriers of obtaining stability and support individual progress while building skills and capacity for everyone. The Program Director will supervise and provide oversight to the Intensive Community Wrap Around Supports team which includes clinicians, nurses, team coaches, consultants, and peer mentors. They will provide trainings necessary for the clinical treatment team including Autism Spectrum Disorder, significant mental illness, evidenced based and promising practices which include PACT, DBT Informed Skills system, Motivational Interviewing, Harm Reduction and PBS. Other Key Responsibilities:
  • Facilitate a daily communication forum for all team members to collaborate, plan and take action.
  • Manage all referrals for the Community Treatment Team to include tracking, responding to the funder, maintaining active rosters and wait lists.
  • Ensure all required clinical assessments are scheduled and completed within 30 days of admission and ensure all service activities provide a combination of in-person and virtual support modalities to meet the comfort of all individuals served.
  • Develop and participate in the after-hours on-call rotation to promote community access and stabilization for individuals served.
  • Submit billing and program expenditure documentation accurately and within required timelines.
    
    Qualifications
    
    
    
    
    • Master's Degree in Social Services field with current licensure (LMHC, LICSW, BCBA or higher) or license eligibility, required.
    • Minimum of five years direct experience with individuals with Autism Spectrum Disorder and Significant Mental Illness, required.
    • Direct experience implementing evidence-based practices, required.
    • Valid driver's license, acceptable driving record and reliable vehicle, required.
